Soil Sample – SUBMISSION

This form is provided for client to submit sample information prior to sending samples so the SHAC team can be prepared when the samples arrive. We will not begin processing samples without this form being submitted. Sample drop off is from 7:30AM to 4PM Monday through Friday.

Per the USDA APHIS PPQ it is the responsibility of the sender of samples to determine if they are in an APHIS regulated area. The following APHIS USDA Website can be used as a guide. If you are unsure please contact APHIS.

We are not permitted to accept samples from the following APHIS regulated areas due to fruit flies and pale cyst nematodes:

  • Texas: Cameron, Hidalgo, Starr, Zapata (new as of 04/01/24)
  • California: Contra costa, Santa Clara
  • Idaho: Bingham, Bonneville
  • New York: Orleans, Wayne, Erie, Monroe, Niagara, Ontario

If your soil is APHIS regulated please see our APHIS shipping instructions and Compliance Agreement for samples from the 48 contiguous United States. The Compliance Agreement must be in your shipment and your package clearly marked as APHIS. For international soils (including Hawaii, Pueto Rico and Alaska) you must use international shipping instruction, use shipping labels and include the 525 permit in your shipment.

If this date is sooner than 4 months from sample arrival — it is a goal we will do our best to meet, but is not a deadline without expediting.
Do your samples need to be expedited? We do not expedite projects larger than 150 samples.
Expedited samples are subjected to a 25% fee and roughly 3-5 week turn around from sample arrival depending on sample count (30 samples = 3 weeks, 60 samples = 4 weeks, 100-150 samples = 5 weeks). Your sample analyses will be completed by the date provided and data report issued soon after. If your date is not feasible we will contact you shortly after your submission or your samples are received.

If you are told your file is spam please submit your form anyways. You will be prompted that you are missing fields. Reattach your spreadsheet and submit again. You should submit successfully on the second try and end with a “Thank you” page. We apologize for the inconvenience and are working with the web designers.
